The survey does exist and no, you don’t have to be a subscriber. When it’s ready for you to respond, it will show up on the lower right menu, between the Premium and Clans buttons. Oh, and this is on the main map only, not in an individual level screen.

I play on two accounts, one subscribed and the other not. I did notice that my main account stopped seeing them, but I was still seeing them on my other…I figured my main had reached the limit, as the questions had started repeating. However, I did get the survey pop on my other, yesterday afternoon. If they are going to show, the icon will appear hear, between the Premium and Clans buttons:
