So, I have been reading sever suggestions and people have given hints to the level 66 items and so on will these be actually be incorporated into the game and which will remain as concepts?
And what about the level with question marks on it? Whats that all about?

No, if you read Suggestions Thread, everywhere (or almost everywhere) if someone answer about abilities of not released heroes, the answer was you will know when it release, or something like this.

the volcano worls will be added in time
More levels are going to be added so that level 66 is actually achievable for non-staff members. Then you can unlock all the items and buy them for a price. The ??? world is the volcano world, which isn’t going to come out anytime soon since there are gong to be more glacier levels first
