Missing Levels?

Is there anyway to find out why certain levels are not unlocked or not appearing in some campaigns?

Currently, the following levels are locked or missing altogether:

Kithgard – Sleeping on the Job – currently locked

Sarven Desert – Golden Mirage & Goalkeeper – missing/not shown

I cannot access them directly by entering the URL either.

I see that others have previously asked here for help with this issue and not received any response. Is this a bug and should it be submitted to github?

Golden Mirage and Goalkeeper must be unlocked in a different world. Keep playing to unlock it.

Sleeping on the Job works for me, play True Names again. It is a subscriber level.
